The Journey of Gestation
A standard human pregnancy lasts approximately 280 days (40 weeks) from the first day of the last menstrual period. Understanding the milestones and physiological requirements of each stage is vital for maternal and fetal health.
Precision Dating Methods
There are several clinically accepted ways to estimate a pregnancy's timeline. While the **Last Menstrual Period (LMP)** is the most common, it assumes a standard 28-day cycle with ovulation on day 14.
Ultrasound Dating
The most accurate method for dating, especially in the first trimester. A technician measures the crown-rump length (CRL) to determine the exact gestational age within a few days of accuracy.
IVF & Conception
When the date of conception or embryo transfer is known exactly, the due date is calculated as 266 days from fertilization (which equates to 280 days from a theoretical LMP).
The Three Trimesters
Pregnancy is traditionally divided into three stages called trimesters, each about 13-14 weeks long. Each stage brings unique physical and emotional changes.
First Trimester
The foundational period where all major organs develop. Common symptoms include morning sickness, fatigue, and frequent urination. By week 12, the fetus is fully formed.
Second Trimester
Often called the 'Golden Period.' Most early symptoms fade. The baby begins to move, hear external sounds, and gender is often visible via ultrasound around week 18-20.
Third Trimester
The final stretch. Focus is on weight gain and lung maturation. As the baby grows, the mother may experience backaches and Braxton Hicks contractions.
Fetal Development Milestones
| Gestation | Key Development | Clinical Significance |
|---|---|---|
| Week 3 | Conception | Fertilization occurs; DNA is combined. |
| Week 4 | Positive Test | HCG levels detectable in urine/blood. |
| Week 6 | Heartbeat | Cardiac activity detectable via ultrasound. |
| Week 12 | Fetal Transition | Organs are formed; risk of miscarriage drops. |
| Week 20 | Quickening | Movement felt; Anatomy scan performed. |
| Week 24 | Viability | Lungs begin producing surfactant. |
| Week 37 | Early Full Term | Organs ready for life outside the womb. |
| Week 40 | Due Date | Expected date of delivery (Naegele's Rule). |
Clinical Screening & Safety
Drug Categories
The FDA classifies drugs based on risk. Category A is safe; Category X is strictly prohibited during pregnancy.
Standard Weight Gain
Normal BMI (18.5-24.9) targets: 25-35 lbs. Overweight (25-29.9) targets: 15-25 lbs.
Ultrasound Timeline
Dating scan at week 6-8; Anatomy scan at week 18-20 to monitor structural development.
Preeclampsia Watch
Monitor for sudden weight gain, severe headaches, or vision changes which may indicate high blood pressure.
